HOW TO ENTER THE 2009 QUAD CITIES SENIOR OLYMPICS
1. Complete the entry form: complete all information on both pages of the form. Send
it to: Quad Cities Senior Olympics, Inc., 1800 3rd Avenue, Suite 304, Rock Island, IL 61201.
2. Be in good physical condition and sign the liability WAIVER STATEMENT.
3. Be 50 (40 for track and field, 5 k and swimming) years of age by June 25, 2009.
4. Include total payment by check or money order made payable to: Quad Cities Senior Olympics, Inc.
5. Olympic Games, Senior Challenge, Performing Arts registrations that are received by June 17th, 2009 will be confirmed. After June
17th, 2009, registrations will be accepted if openings are available.

REGISTRATION:
Individuals or groups of two must register individually.
A one-time registration fee of $15 ($20 after 6-17-09) entitles each participant to enter any and all events if received by June 17th.
Groups of three or more may choose to participate under a $40 fee. This is to be paid by the leader only and entitles group members to
participate in that group event ONLY. You may register each member of the Group as an individual. If this option is selected,
ALL MEMBERS of the group MUST be individually registered. However, each group member beyond two must pay $8 to cover cost
of T-shirt. Additional on-site facility fees are required for golf ($14), cart ($13), bowling ($5), and billiards ($3).
NEW: Track & Field. Swimming and the 5K Road Race will open its entries to the 40-44 and 45-49 age groups this year. They
will compete for medals in their own age groups. Cornbelt Running Club members have been offered a two for one entry
price for the 2009 games. All Track & Field entries will be limited to earning 3 medals. After the limit, they will pay $5.00 for
each additional medal earned. Results will be sent to the Evald Headquarters immediately at the conclusion of each event. All
medals earned in Track & Field and the Road Race will be picked up at Evald Hall Headquarters.
AWARDS:
Athletic Events
A Gold, Silver or Bronze medal will be awarded to the top three competitors of each event and may be picked up at the Olympic
Headquarters at the Evald Hall. Augustana College (awards will not be mailed). The awards table at the Olympic Headquarters will be
open Friday 4 PM - 9 PM, Saturday 8 AM - 5 PM. Medals for sports events will be awarded to men and women in five year age group
increments beginning at age 50.
Senior Challenge & Senior Arts
A Gold, Silver or Bronze medal will be awarded to the top three competitors of each event at the Arts Reception
(awards will not be mailed).

SERVICES
Those participating June 26-27, except golfers, must check in at Headquarters prior to participation. (Golfers check in at golf course.)
Services Include:
1. Registration packets for ALL registrants except golfers.
2. Directions, motel and restaurant lists, and answers to questions.
3. Meeting and mingling with other participants.
4. Complimentary light refreshments.
5. Picking up medals, getting medals engraved, etc., if applicable.
6. Enjoying opening ceremonies June 26 and closing ceremonies and complimentary meal June 27th.
7. Visiting with vendor businesses on June 27.
8. Picking up T-Shirts for participants.
CLOSING CEREMONIES/COMPLIMENTARY Meal:
Complimentary meal at 4 pm, Saturday, June 27, at
Ericson Stadium following closing ceremonies.
CANCELLATION/POSTPONEMENTS:
In the event of inclement weather or unusual/ extenuating circumstances, Quad Cities Senior
Olympics, Inc. officials reserve the right to cancel or postpone events.
