- Be in good physical condition and SIGN the LIABILITY WAIVER STATEMENT
- Be 50 years of age (40 for swimming, track & field, and 5K Run and Walk) by June 21, 2011
- Include entry fee payment of $15 by check or money order to Quad Cities Senior Olympics, Inc, by June 21, 2011. DO NOT SEND CASH. On site facility fees are required for golf $15, golf cart $14, bowling $6 .75, and billiards, $3.
- A donation of $3 per medal in excess of five (5) medals is requested.

AWARDS
CHECK-IN for participant packets, hospitality, and pick up of on-site medals will be at Headquarters (Bergendoff Hall) Augustana College, 3701 7th Avenue, Rock Island IL. Headquarters is open Friday, June 24th 2PM - 9 PM, and Saturday, June 25th7:30AM - 5 PM.
Literary and Arts & Crafts Events: Gold, Silver, Bronze medals will be awarded at the Arts Reception on Wednesday. June 22, 2011, 7:00 PM at CASI, 1035 W Kimberly Rd, Davenport IA.
Performing Arts: *NOTE NEW TIME - 2:00 PM* Medals will be awarded after the completion on Thursday, June 23, 2011, at 2:00 PM at the Tri-City Jewish Center, 2715 30th Street, Rock Island IL.
Off-site event medals will be awarded at the events site. Athletic and other event medals not awarded at the event site , must be picked up at Headquarters (Bergendoff Hall). Medals for athletic events will be awarded to men and women in five year increments beginning at age 50 (age 40 at specific events).
SERVICES: All registrants participating in on-site events June 24-25 must check in at Headquarters (Bergendoff Hall) to pick up their participant packet. For participants in events prior to June 24, the packets will be picked up at the participant's first event site.
SERVICES AT OLYMPIC HEADQUARTERS (BERGENDOFF HALL) INCLUDE:
- Packet pick-up for all events held at Augustana. All events held away from Augustana, packet pick up is at the event location. Please note that t-shirts must be picked up at Headquarters by participant.
- Directions, motel and restaurant lists, and answers to questions.
- Meeting and mingling with other participants.
- Complimentary light refreshments
- Picking up medals, getting medals, engraved, etc
- Enjoying the June 24th Opening Ceremonies, and the June 25th Closing Ceremonies and Complimentary Meal
- Visiting with vendor business on June 25th
- Picking up t-shirts CLOSING CEREMONIES/COMPLIMENTARY MEAL: Complimentary meal at 4 PM on Saturday, June 25th atEricson Stadium followed by closing ceremonies.
CANCELLATIONS/POSTPONEMENTS: In the event of inclement weather, or unusual/extenuating circumstances, Quad Cities Senior Olympics, Inc, officials reserve the right to cancel or postpone events.
