HOW TO ENTER THE 2010 QUAD CITIES SENIOR OLYMPICS
1. Complete the entry form: complete all information on both sides of the form. Send to Quad Cities Senior Olympics, Inc, 1800 3rd Avenue, Suite 304, Rock Island IL 61201
2. Be in good physical condition and sign the liability WAIVER STATEMENT
3. Be 50 years of age (40 for track and field, 5K, and swimming) by June 22, 2010
4. Include payment by check or money order to Quad Cities Senior Olympics, Inc for t-shirt ordered
5. Registrations for the Olympics Games, Senior Challenge, and Performing Arts received by June 16, 2010 will be confirmed.

FEES: ZERO ENTRY FEE for the 20th Anniversary Games. T-shirts may be ordered at a cost of $9 each. On site facility fees are required for golf, $15, cart, $14, bowling, $6, and billiards, $3. All entries will be limited to earning 5 medals. A donation of $3 per medal in excess of 5 medals is requested.
AWARDS
Arts Events: A Gold, Silver, or Bronze medal will be awarded to the top three competitors of each event (Literary, Visual and Heritage) at the Arts Reception on Wednesday, June 23, 2010, at 7 PM at CASI, 1035 W Kimberly, Davenport IA.
Performing Arts: Medals will be awarded after the competition on Thursday, June 24, 2010, at 7:00 PM at the Tri-City Jewish Center, 2715-30th
Street, Rock Island, IL
Athletic and other events medals will be picked up at the event site. Medals for athletic events will be awarded to men and women in five-year Increments beginning at age 50.
CHECK-IN for participant packets, hospitality, and pick up of awards/medals at Olympic Headquarters Carver Center, Augustana College, 3500 5th Avenue, Rock Island IL. Open: Friday, June 25, 3 PM – 9 PM, Saturday, June 26, 2010, 8 AM – 5 PM.
SERVICES: All registrants participating June 25-26 must check in at Headquarters to pick up their participant packet unless they have designated a specific event manager to pick up their packet. For participants in events prior to June 25, the participant should designate a specific
event manager to pick up his/her packet.
Services at Olympic Headquarters include:
1. Packet pick up at Carver Center for all events held at Augustana. All events held away from Augustana, packet pick up is at the event location. Please note that T-shirts ordered must be picked up at Headquarters by participant.
2. Directions, motel and restaurant lists, and answers to questions
3. Meeting and mingling with other participants
4. Complimentary light refreshments
5. Picking up medals, getting medals engraved, etc, if applicable
6. Enjoying the June 25th opening ceremonies and June 26th closing ceremonies and meal
7. Visiting with vendor businesses on June 26
8. Picking up T-shirts ordered at Headquarters by participant
CLOSING CEREMONIES/COMPLIMENTARY MEAL: Complimentary meal at 4 PM Saturday, June 26 at Ericson Stadium followed by closing ceremonies
CANCELLATIONS/POSTPONEMENTS: In the event of inclement weather or unusual/extenuating circumstances, Quad Cities Senior Olympics, Inc, officials reserve the right to cancel or postpone events.
